Our mission is to provide a range of services, including short-term rehabilitation, intermediate care, skilled nursing and ventilator supported care in a community setting. We strive to create innovative activity-based programs which encourage social interaction and attend to the special needs of our residents and their families in a compassionate and caring manner. We honor the dignity and independence of each resident while providing a safe, secure, and healthful environment.
CCOH works with prospective residents and their families to ease the transition to our community. Our experienced nursing team provides compassionate care to our residents around the clock. Our staff is comprised of various disciplines including nursing, social services, dietary, rehab services and activities. These trained professionals, along with the Attending Physician, work with each resident and their family members to develop an individualized plan of care.

- Accommodation in a private, semi-private, or multiple occupancy room
- Assistance with bathing, dressing, personal hygiene and grooming
- Medication management and administration
- General nursing care and assessment
- Three nutritious meals, plus snacks
- Social Work services
- On-site recreational and therapeutic activities
- Housekeeping and linen services (7 days a week)
Admission
All residents are admitted and are attended to by their personal physicians. Caring staff and social workers coordinate the admission process to the facility and address any concerns during the transition period. Personal and/or family counseling is provided throughout the residents’ stay.
Room Assignment
Special care is taken by the Center's skilled Social Workers to place residents with socially, medically, and physically compatible roommates. For those preferring more privacy, single occupancy rooms may be available.
