Nursing Supervisor, RN - Full Time

Primary Responsibilities

Under the general direction of the Director of Nursing, is responsible for the operational activities of the facility. The Nursing Supervisor, RN acts as a liaison between the Director of Nursing and staff and is responsible with twenty-four (24) hour accountability for compliance of appropriate nursing care standards in the planning, organizing, directing and implementation of nursing care. In liaison with the Director of Nursing, is directly responsible to achieve and maintain quality resident care in a cost-effective manner.

 

Essential Job Functions
  • Manages the daily operational activities of the facility:
  • Applies the facility’s philosophy, objectives, and policies as appropriate with nursing staff, residents, and visitors.
  • Communicates pertinent information to staff; keeps Director of Nursing informed of significant activities
  • Assists with ensuring staffing requirements are met.
  • Complies with internal and external standards
  • Evaluates assigned Facility supply/equipment needs and maintains an inventory necessary to meet resident care needs.
  • Manages overall planning, organization, direction, and implementation of nursing care in accordance with policy and procedure, State and Federal regulations to promote high quality care and service.
  • Consults with Human Resources on application of Employee Handbook policies especially when progressive discipline of a union employee as certain rules applies.
  • Ensures all licensed nurses on the Facility are thoroughly familiar with use of AED equipment.
  • Collaborates with healthcare team (both internal and external) about resident care:
  • Investigates, collaborates, and problem solves with physicians and other departments as necessary; reviews and follows up on incident reports.
  • Facilitates problem solving with appropriate resolution(s).
  • Attends resident care conferences.
  • Facilitates/collaborates discharge planning.
  • Promotes performance improvement activities:
  • Provides appropriate orientation for new staff; ensures that staff possesses knowledge about emergency procedures.
  • Ensures staff compliance with safety requirements of the Unit; encourages staff attendance to all mandatory in-services and continuing education classes.

  • Daily Tasks
  • Reviews 24 hour report to verify

– MD/RP notified of changes in condition;

– Care plans are completed for any change in condition;

– Follow through on all physicians’ orders;

– Schedule consults as order by physicians;

– Obtain labs as ordered and in accordance with plans of care (PT/INR, Dig. Level, UA, etc.);

– Complete infection surveillance log as appropriate;

  • Attends clinical stand up meeting.
  • Gives report to the incoming shift supervisor.
  • Audits charts routinely including documentation in Point Click Care for completion and accuracy.
  • Meets newly admitted residents.
  • Reviews weekly summaries and care plans.
  • Reviews discharge charts.
  • Makes daily rounds with staff (LPNs, CNAs, RNAs, RNs) to check

– Devices are in place per plan of correction;

  • Reviews ADL documentation for accuracy 2-3 weeks before the net MDS is due.
  • Supervises staff to ensure timely meal delivery and uninterrupted feeding assistance to enhance the residents dining experience.
  • Immediately addresses all resident concerns and reports next day at daily stand up.
  • Time and Attendance

– Reviews daily staffing and schedule before 9am;

– Reviews hours in Onshift to track tardiness, early clock-ins; late clock-outs, absenteeism;

– Uniformly applies Employee Handbook guidelines.;

– Identifies reasons for overtime;

– Reviews timesheets daily to monitor time clock practices; attendance, tardiness and sick time;

– Responsible for PPD labor management of the daily productive hours facility be within budget;

– Limits use of overtime hours.

  • Ensures all empty beds are neat, tidy and ready for admission.
  • Inspects the quality of the work performed by nursing staff, making daily inspection rounds with special attention to infection control, privacy and residents’ rights.
  • Reviews and ensures compliance with all physicians’ telephone orders.
  • Ensures new admits have everything they need.
  • Follows up on all labs.
  • Ensures compliance with HIPAA.
  • Weekly Tasks:
  • Reviews weekly weights for accuracy.
  • Reviews skin assessments and wound assessments for completeness and accuracy.
  • Makes wound rounds with the Charge Nurses to ensure accuracy.
  • Makes wound rounds with the Wound Care, RN and/or doctor.
  • Reviews ADL scores in Point Click Care.
  • Attends PPS/Case Management meetings twice each week when scheduled.
  • Helps determine (per CMS guidelines) whether residents qualify for continued stay for skilled nursing services when skilled therapy services discontinued.
  • Reviews hospice documentation weekly.
  • Attends and actively participates in nursing supervisory meetings.
  • Monthly Tasks:
  • Reconciles physicians’ orders;
  • Reviews each resident’s care plan for completeness and accuracy;
  • Attends Care Plan conferences;
  • Assists the Director of Nursing in following through on all pharmacy recommendations;
  • Actively participates in the quality assurance process, including attendance at QA&PI meetings and submission of required reports;
  • Completes performance appraisals of staff when due.

Respiratory Therapist (RRT/CRT) - Full Time $5,000 Sign on Bonus

Primary Responsibilities

Under the supervision of the Director of Respiratory Care, performs respiratory care procedures in accordance with established policies and procedures, through assessment, formulating respiratory diagnosis, planning and implementing, documenting and evaluating plans of respiratory care to assigned residents.  Complies with and enforces VCU/RCU policies and regulatory requirements.  Therapist is expected to be self directed in the absence of the Respiratory Director.

 

Essential Job Functions
  1. Performs all respiratory care procedures according to VCU/RCU policy and procedures, including but not limited to mechanical ventilation, oxygen therapy, aerosol therapy, bronchopulmonary drainage, plus oximetry, and tracheostomy care.
  2. Evaluates effects of respiratory therapy treatment plan by observing, noting, and evaluating patient’s progress; recommending adjustments and modifications.  Documents patient care services by charting in PointClickCare once per shift.
  3. Provides rapid intervention of respiratory care in order to prevent medical crisis and discharge of the resident from the facility.
  4. Verifies physician’s orders before administration of therapy.  Clarifies, modifies and renews orders as needed.
  5. Informs physicians and other health team members of the patient’s condition, problems or concerns in a prompt and effective manner.
  6. Provides complete report to oncoming personnel and takes a complete report from off going personnel.
  7. Performs education and support on all respiratory care equipment to resident, families and staff.
  8. Operates, inspects, assembles and maintains respiratory care equipment and supplies.  Performs routine maintenance, troubleshooting and reports malfunctioning equipment immediately to Unit Manager and/or Program Manager.
  9. Contributes to team effort by accomplishing related duties as needed/assigned.

  1. Maintains level of professionalism by attending all required meetings/in-services and completing annual competency requirements.
  2. Assists in the collection and communication of data required or complete and accurate submission of MDS assessments.

Primary Responsibilities

Under the supervision of the Unit Manager, Nursing Supervisor, RN and/or LPN, the certified nurse aide (CNA) assists in the care of residents.  Duties involve a variety of patient care activities, including routine/selected nursing procedures and related non-professional services devoted to the personal needs and comfort of residents.

  1. Provides general nursing care to meet the medical, physical and social needs of assigned residents in a respectful manner and provides necessary care or services as requested.
  • Answers signal lights to determine resident needs.
  • Assists residents with activities of daily living.
  • Provides comfort measures as directed.

  • Sees to personal hygiene; elimination needs of the residents.

  1. Participates in the interdisciplinary comprehensive assessment process in evaluating residents’ functional status, and emotional, social and medical therapeutic needs and in maintaining the residents’ maximum potential for independence.

General Summary:

The Registered Nurse preforms assessments, planning and implementation of care to assigned Residents in accordance with professional nursing practice standards, Federal and State regulations and CCOH policies and procedures. May monitor, assign and/or evaluate care provided by licensed or non-licenses health care staff.

Essential Job Duties and Responsibilities:

  1. Participates in the selection of licensed and non-licensed staff. Conducts training, manages performance and contributes to a culture of excellence in service, compliance and care
  2. Responsible Assessment and monitoring of Resident conditions, Infection Control systems, psychopharmacological medications management, proper use of restraints, foley catheters as prescribed. Maintains constant systems to check for, report and reduce pressure ulcers, Input/Output.
  3. Conducts regular Resident Chart audits-includes but is not limited to; Weekly summaries and care plans, identifies issues, takes corrective action and follows up on system, operations issues. Reviews EMRs and TARS, Physician Summaries
  4. Represents the Department of Nursing in (includes but is not limited to): Stand Up, PPS, QA&A, Resident Care Conferences

  2. Performs Active Range of Motion (AROM) and Passive Range of Motion (PROM) in a safe manner to residents as assigned by the Unit Manager and the resident’s overall plan of care.
  3. Ambulates residents according to plan of care while utilizing appropriate equipment or devices in a safe manner.

May assists residents in applying or removing splints or prostheses.

  1. Encourages residents to perform tasks for him/her as appropriate.
  2. Provides good body alignment, positioning device and contracture padding as appropriate.
  3. Reports adverse conditions to the Unit Manager.
  4. Monitors and assists with the performance of Activities of Daily Living (ADLs) to promote resident involvement.
  5. Evaluates and documents resident’s progress and communicates results to the Unit Manager.
  6. Cooperates with licensed rehabilitative therapists when providing restorative nursing care to residents when appropriate.

Other Duties

  1. Accompanies residents to various facilities, clinics, etc., if needed.
  2. May receive instructions from resident and/or family member regarding resident care. Reports requests to RN or LPN.
  3. Performs other related duties as assigned.

Skills/Knowledge:

  1. Good communication and organizational skills with the ability to handle multiple tasks with minimal supervision.
  2. Ability to write legibly and read and understand the English language sufficiently to perform the duties of the position.
  3. Ability to demonstrate organized, disciplined approach to completing work and to build and maintain cooperative working relationships with others.
  4. Mathematical ability and accuracy required to calculate intake/output measurements.
  5. Skills necessary to operate computer.

Experience:

  1. Recent experience (within the past two (2) years) in an acute care or skilled nursing facility preferred.

License/Certification:

  1. Graduate from a Restorative Aide Program with Certification.
  2. Must possess a State of Hawaii Certified Nurse Aide Certification.
  3. Current BLS Health Care Provider certification; must be physically able to perform CPR.

Dietary Aide - Full Time

Primary Responsibilities
Under the direct supervision of the Assistant Food Service Manager/Director of Dietary Services, is responsible for setting
up food trays for residents. Prepares between meal nourishment, salads or other foods and may work cold or hot stations
on the central tray services line. Is also responsible for a variety of cleaning tasks in the kitchen.
Essential Job Functions
1. Portions and prepares juices, fruits, food for tray services.
2. Prepares and delivers tube feeding to units.
3. Prepares and delivers meal nourishment to the units.
4. Serves trays or “HOLD” trays.
5. Services residents and employee dining room.
6. Updates daily diet census.
Other Duties
1. Cleans work areas. Sweeps, mops and washes floors and mats.
2. Strips food trays to prepare for dishwashing.
3. Washes pots and pans, loads and unloads dishes from dish machine.
4. Follows proper safety and sanitation techniques.
5. Performs other related duties as assigned.
Minimum Qualification Requirements:

2 Courtesy and tact in dealing with residents, visitors and employees.
